We take on the psychotherapy portion of the experience, while Journey Clinical’s medical team or Empathy MindCare supports you on all medical aspects. This includes determining eligibility, developing a custom treatment plan, prescribing the medicine and monitoring outcomes. Ketamine Assisted Psychotherapy (KAP) is a form of Psychedelic Assisted Psychotherapy, which is being studied within institutions such as Johns Hopkins , NYU, Stanford, Imperial College of London, Usona Institute, and the Multidisciplinary Association for Psychedelic Studies. Some research universities, including Yale, have created specialized groups of clinicians and scholars who gather regularly to discuss psychedelic medicine. One such groups, the March 2021 Grand Rounds, featured Roland Griffiths from Johns Hopkins presenting on the neuropsychopharmacology of psilocybin.

Ketamine is a legal, safe and effective medicine used to treat a variety of mental health conditions, including depression, anxiety and PTSD. Ketamine has rapidly-acting antidepressant and mood-enhancing effects, which can begin to take effect within 1-2 hrs. after treatment and last for up to 2 weeks. It works by blocking the brain’s NMDA receptors as well as by stimulating AMPA receptors, which are thought to help form new synaptic connections and boost neural circuits that regulate stress and mood. Ketamine has also been shown to enhance overall neuroplasticity for lasting symptom improvement.
Ketamine can be administered in a variety of ways, including IV infusion, intramuscular injection, via nasal spray and using sublingual lozenges. In our work with Journey Clinical we only use the sublingual lozenge form (oral tablets) or nasal spray with Empathy MindCare.
Ketamine has been used as an anesthetic since 1970. At lower doses, it can act as a dissociative psychedelic. Studies reveal ketamine’s antidepressant effects with FDA approval of eskatamine/Spravado (a nasal spray) for refractory (treatment resistant) depression in 2019.
Currently, ketamine is being used off-label to treat an array of psychiatric diagnoses. In recent years, ketamine infusion clinics have cropped up across the country. This IV (infusion) model differs from KAP and this distinction is described eloquently by Dr. Raquel Bennett, founder of KRIYA Institute. In short, KAP’s effectiveness derives mainly from psychological effects facilitated by an altered state of consciousness within a trusting therapeutic relationship. It is my belief that this inner transformation gets to the root of suffering (i.e., accesses our innate trove of healing intelligence).
The effects of ketamine, which most patients find pleasant, last for approximately 45 minutes. These effects can make you feel “far from” your body and facilitate shifts in perception that can often feel expansive in nature. Your motor and verbal abilities will be reduced, so you’ll be lying down in a comfortable position during the experience. Once these effects subsided, we’ll spend the remainder of our appointment giving you space to process and discuss your experience. While it may feel hard to articulate what happened during the experience, patients feel like the insights gained are none-the-less clear. Studies have shown that the benefits to mood and neurological growth can last up to two weeks after the Ketamine experience.
Pivot Counseling uses a structured format based on evidence-based practices such as Acceptance Commitment Therapy, Cognitive Behavioral Therapy, and Solution Focused Brief Therapy. Each KAP segment will include Preparation, Dosing, and Integration:
Preparation Session:
The preparation session is designed to set clear intentions and identify personal goals before beginning the dosing and integration process. The goal is to ensure you are mentally primed and focused, creating a strong foundation for meaningful insights. This session may include guidance on optimizing your setting for the best therapeutic outcomes, as well as instruction on key skills and techniques to support your experience during and after the dosing session.
Dosing Session:
The dosing session follows a structured, carefully designed protocol to ensure safety, consistency, and effectiveness. Each step is clearly outlined, including how to prepare for dosing, what to expect during the experience, and the roles of both the therapist and client throughout the process. We also provide specific guidance on how to conclude the session, ensuring a smooth transition into the integration phase.
Integration Session:
In the integration session, we revisit the intentions, goals, and skills established during the preparation phase, now viewed through the lens of the dosing experience. The focus is on processing insights, reinforcing positive shifts, and deepening motivation for meaningful change. This session helps clients make insightful connections, apply their experiences to real life, and refine the mindsets and skills necessary for lasting transformation.
Journey Clinical is a platform for licensed psychotherapists to incorporate science-based psychedelic therapies in their practice safely and effectively, starting with Ketamine- Assisted Psychotherapy (KAP). Journey Clinical’s in-house medical team takes on patient eligibility, prescriptions and outcome monitoring, while we take on the therapy. Their collaborative care model is designed to deliver personalized treatment plans to meet your individual needs and improve long-term outcomes.
Initial consultation with Journey Clinical
● You schedule an initial evaluation with a clinician from the Journey Clinical medical team via zoom. They will go over your medical and psychiatric history with you, provide education on the treatment and determine if you are eligible for KAP.
● If Journey Clinical’s medical team determines that you are eligible for KAP, they will develop a personalized Ketamine prescription and outcome monitoring plan for you.
● Journey Clinical’s medical staff will write a ketamine prescription for you, and a small amount of oral ketamine will be sent to your home, enough for the first 2 KAP sessions. You will be taught to take your vitals and self-administer the ketamine lozenges by Journey Clinical’s medical team in advance of our KAP sessions.
2. Preparation sessions:
● Once you receive your ketamine lozenges, we will schedule time together for our KAP preparation, dosing and integration sessions. Preparation session(s) will be scheduled just like regular therapy sessions prior to the KAP dosing session. The goal of a preparation session(s) is to align on the process and set intentions for our KAP sessions together.
3. KAP Dosing Session:
● A typical ketamine dosing session lasts between 1-2 hours and can take place either in-person at my office, or remotely via telehealth.
● During a dosing session, you will self-administer your ketamine lozenge either in our office or in your home. You will be in a comfortable, reclining position wearing an eye mask and listening to calming music. Although a KAP dosing session may be largely an internal experience, we will be present with you the entire time to hold space and provide support as needed.
4. Integration Sessions:
● After our KAP dosing session, we will meet for multiple integration therapy sessions to review the memories, thoughts & insights that arose during your dosing session, and to prepare for the next dosing session.
5. Follow-up consultations with Journey Clinical:
● After our first KAP session, Journey Clinical’s medical team schedules regular follow ups with you to monitor outcomes and prescribe ketamine lozenge refills, as appropriate. The frequency of follow ups depends on your unique treatment plan, at a minimum of once per quarter.

Ketamine-Assisted Psychotherapy is a specialty treatment that creates opportunities for breakthrough in a shorter amount of time compared to psychotherapy alone. This will involve monitoring for both medical and mental aspects of treatment. The cost is in reference to the counseling portion of treatment:
KAP Initial Intake: $400: Intake interview to gather background information and eligibility for Ketamine treatment. If you decide to move forward with the KAP package, this initial fee will be subtracted from the total rate of the package.
KAP Dosing Package: $2500
Preparation Session: Gain clarity about the breakthrough goals, acquire skills, and general setup before dosing session (1 hour).
Dosing Session: Preparation for dosing (vital check, setting setup), dosing with Ketamine, and concluding tasks (2-3 hours).
Integration Session: Further discussion of the Ketamine experience and ways to integrate it to facilitate healing and breakthrough (1 hour).
