How To Choose The Right Family Therapist For Your Needs
Family dynamics can be intricate, with unique challenges that evolve. Finding the right family therapist is a critical step toward fostering greater understanding, improving communication,

Every journey begins with understanding.
Ketamine-Assisted Psychotherapy (KAP) is a holistic modality in which ketamine is used as a complement to psychotherapy to help eligible patients experience more frequent breakthroughs and sustained improvement in symptoms. We take on the psychotherapy portion of the experience, while Journey Clinical’s medical team or Empathy MindCare supports you on all medical aspects. This includes determining eligibility, developing a custom treatment plan, prescribing the medicine and monitoring outcomes. Below is more information about KAP to help you navigate if it may be a good fit for you.
Ketamine Assisted Psychotherapy (KAP) is a form of Psychedelic Assisted Psychotherapy, which is being studied within institutions such as Johns Hopkins , NYU, Stanford, Imperial College of London, Usona Institute, and the Multidisciplinary Association for Psychedelic Studies. Some research universities, including Yale, have created specialized groups of clinicians and scholars who gather regularly to discuss psychedelic medicine. One such groups, the March 2021 Grand Rounds, featured Roland Griffiths from Johns Hopkins presenting on the neuropsychopharmacology of psilocybin.
At Pivot Counseling, our Ketamine-Assisted Psychotherapy (KAP) sessions are led by certified therapists who prioritize your comfort, safety, and healing. We provide personalized support in a nurturing and judgment-free space, ensuring that your experience is both transformative and deeply meaningful.
True healing happens in the right environment. Our luxurious, tranquil therapy spaces in Walnut Creek and Dublin are designed for comfort and peace, allowing you to explore your healing journey with undivided attention and expert guidance. Here, you are treated like family, with care that is as personal as it is professional.
We go beyond ketamine treatment—we provide integrative, evidence-based therapy to ensure that your healing extends into everyday life. By combining cutting-edge science with compassionate care, we help you experience lasting relief from depression, anxiety, PTSD, and more, leading to real and lasting transformation.

Ketamine is a legal, safe and effective medicine used to treat a variety of mental health conditions, including depression, anxiety and PTSD. Ketamine has rapidly-acting antidepressant and mood-enhancing effects, which can begin to take effect within 1-2 hrs. after treatment and last for up to 2 weeks. It works by blocking the brain’s NMDA receptors as well as by stimulating AMPA receptors, which are thought to help form new synaptic connections and boost neural circuits that regulate stress and mood. Ketamine has also been shown to enhance overall neuroplasticity for lasting symptom improvement.
Ketamine can be administered in a variety of ways, including IV infusion, intramuscular injection, via nasal spray and using sublingual lozenges. In our work with Journey Clinical we only use the sublingual lozenge form (oral tablets) or nasal spray with Empathy MindCare.
Ketamine has been used as an anesthetic since 1970. At lower doses, it can act as a dissociative psychedelic. Studies reveal ketamine’s antidepressant effects with FDA approval of eskatamine/Spravado (a nasal spray) for refractory (treatment resistant) depression in 2019.
Currently, ketamine is being used off-label to treat an array of psychiatric diagnoses. In recent years, ketamine infusion clinics have cropped up across the country. This IV (infusion) model differs from KAP and this distinction is described eloquently by Dr. Raquel Bennett, founder of KRIYA Institute. In short, KAP’s effectiveness derives mainly from psychological effects facilitated by an altered state of consciousness within a trusting therapeutic relationship. It is my belief that this inner transformation gets to the root of suffering (i.e., accesses our innate trove of healing intelligence).
The effects of ketamine, which most patients find pleasant, last for approximately 45 minutes. These effects can make you feel “far from” your body and facilitate shifts in perception that can often feel expansive in nature. Your motor and verbal abilities will be reduced, so you’ll be lying down in a comfortable position during the experience. Once these effects subsided, we’ll spend the remainder of our appointment giving you space to process and discuss your experience. While it may feel hard to articulate what happened during the experience, patients feel like the insights gained are none-the-less clear. Studies have shown that the benefits to mood and neurological growth can last up to two weeks after the Ketamine experience.
Pivot Counseling uses a structured format based on evidence-based practices such as Acceptance Commitment Therapy, Cognitive Behavioral Therapy, and Solution Focused Brief Therapy. Each KAP segment will include Preparation, Dosing, and Integration:
Preparation Session:
The preparation session is designed to set clear intentions and identify personal goals before beginning the dosing and integration process. The goal is to ensure you are mentally primed and focused, creating a strong foundation for meaningful insights. This session may include guidance on optimizing your setting for the best therapeutic outcomes, as well as instruction on key skills and techniques to support your experience during and after the dosing session.
Dosing Session:
The dosing session follows a structured, carefully designed protocol to ensure safety, consistency, and effectiveness. Each step is clearly outlined, including how to prepare for dosing, what to expect during the experience, and the roles of both the therapist and client throughout the process. We also provide specific guidance on how to conclude the session, ensuring a smooth transition into the integration phase.
Integration Session:
In the integration session, we revisit the intentions, goals, and skills established during the preparation phase, now viewed through the lens of the dosing experience. The focus is on processing insights, reinforcing positive shifts, and deepening motivation for meaningful change. This session helps clients make insightful connections, apply their experiences to real life, and refine the mindsets and skills necessary for lasting transformation.
Journey Clinical is a platform for licensed psychotherapists to incorporate science-based psychedelic therapies in their practice safely and effectively, starting with Ketamine- Assisted Psychotherapy (KAP). Journey Clinical’s in-house medical team takes on patient eligibility, prescriptions and outcome monitoring, while we take on the therapy. Their collaborative care model is designed to deliver personalized treatment plans to meet your individual needs and improve long-term outcomes.

Initial consultation with Journey Clinical
● You schedule an initial evaluation with a clinician from the Journey Clinical medical team via zoom. They will go over your medical and psychiatric history with you, provide education on the treatment and determine if you are eligible for KAP.
● If Journey Clinical’s medical team determines that you are eligible for KAP, they will develop a personalized Ketamine prescription and outcome monitoring plan for you.
● Journey Clinical’s medical staff will write a ketamine prescription for you, and a small amount of oral ketamine will be sent to your home, enough for the first 2 KAP sessions. You will be taught to take your vitals and self-administer the ketamine lozenges by Journey Clinical’s medical team in advance of our KAP sessions.
2. Preparation sessions:
● Once you receive your ketamine lozenges, we will schedule time together for our KAP preparation, dosing and integration sessions. Preparation session(s) will be scheduled just like regular therapy sessions prior to the KAP dosing session. The goal of a preparation session(s) is to align on the process and set intentions for our KAP sessions together.
3. KAP Dosing Session:
● A typical ketamine dosing session lasts between 1-2 hours and can take place either in-person at my office, or remotely via telehealth.
● During a dosing session, you will self-administer your ketamine lozenge either in our office or in your home. You will be in a comfortable, reclining position wearing an eye mask and listening to calming music. Although a KAP dosing session may be largely an internal experience, we will be present with you the entire time to hold space and provide support as needed.
4. Integration Sessions:
● After our KAP dosing session, we will meet for multiple integration therapy sessions to review the memories, thoughts & insights that arose during your dosing session, and to prepare for the next dosing session.
5. Follow-up consultations with Journey Clinical:
● After our first KAP session, Journey Clinical’s medical team schedules regular follow ups with you to monitor outcomes and prescribe ketamine lozenge refills, as appropriate. The frequency of follow ups depends on your unique treatment plan, at a minimum of once per quarter.
Follow-up Consultations with Journey Clinical:

At Pivot Counseling, we understand that starting ketamine treatment can bring up concerns, especially around losing control. Feeling anxious about stepping into the unknown is completely normal, and our goal is to ensure that you feel safe, supported, and in charge of your healing process at every step.
We tailor each treatment to your comfort level, and one of the key ways we do this is by adjusting the dose based on your needs and preferences.
If you have concerns about losing control, we can start with psycholytic dosing to gently introduce you to ketamine’s effects. This allows you to stay grounded and build trust in the process, knowing that you’re in a safe environment with a supportive therapist guiding you. Over time, as you gain confidence and comfort, we can discuss whether a deeper experience might be beneficial, always keeping the decision in your hands.
At Pivot Counseling, we treat you like family—meaning we prioritize your well-being, emotional safety, and personal preferences throughout your healing journey. If you have any concerns, we’re here to talk through them with you and find the right approach that feels best for you.

At Pivot Counseling, we are committed to offering thoughtful, evidence-based options for individuals who feel stuck despite traditional treatment. For some clients struggling with depression, anxiety, PTSD, or mood disorders, IV ketamine therapy may offer a new pathway forward.
Through our partnership with Prime Youth Aesthetics and Wellness, clients now have access to medically supervised intravenous (IV) ketamine administration in a safe, professional clinical setting.

For clients interested in ketamine-assisted psychotherapy (KAP), IV administration can be coordinated with integration sessions to help translate the experience into lasting emotional change.
Our goal is not simply symptom relief, but sustainable transformation. Ketamine is never presented as a standalone miracle solution. It is part of a broader, intentional treatment plan designed around safety, integration, and long-term growth.
If you are curious whether IV ketamine therapy may be appropriate for you, we invite you to schedule a consultation to explore your options.

Ketamine, originally developed as an anesthetic, has been extensively studied in recent years for its rapid-acting antidepressant and mood-stabilizing effects. Unlike traditional medications that may take weeks to show benefit, IV ketamine can often produce relief within hours to days for certain individuals.
IV administration allows for:
This level of control makes IV ketamine one of the most reliable and studied delivery methods available.

IV ketamine may be considered for individuals experiencing:
For many, ketamine creates a temporary window of increased neuroplasticity — a period where the brain becomes more flexible and responsive to therapeutic work.
While ketamine can be delivered in several forms (oral, intranasal, intramuscular), IV infusion offers distinct advantages:
1. Precision and Predictability
IV dosing allows for controlled, steady administration tailored to the individual.
2. Medical Supervision
Vital signs and patient comfort are closely monitored throughout the infusion.
3. Rapid Onset
Many patients experience meaningful symptom reduction quickly.
4. Enhanced Integration Potential
When paired with psychotherapy (KAP), the experience can deepen insight, emotional processing, and trauma work.

I was seeking a therapist, counselor, psychologist (not sure you call them to be honest, they just help) 🙂 around the Pleasanton/Dublin and I wanted to make sure they were patient, listening, showing empathy and compassion. Dr. Yen did just that. My family was having some challenging family dynamics and he was able to main my high regard and setup the next steps towards a thriving family.

Dr. Yen has been the primary clinician working with my son. Through his transparency and honesty my son and I have developed a trusting bond. Dr. Yen possess a unique ability to connect with individuals on their level. His approach to therapy made him a perfect fit for my son and I. As a parent it was helpful that he possessed the ability to highlight both the desires of my son and myself. Dr. Yen helped us find peace and helped us to rebuild a damaged relationship. I recommend Dr. Yen to all who seek support, guidance, and understanding of all relationships in life.

I have nothing but great things to say about having Tim as my therapist. As a female, I was hesitant to work with a male psychologist but I'm glad that I took the chance to do so. Tim is unlike any therapist I have worked with. He is more relatable and someone who is easy to talk to. He has a more outgoing personality and doesn't just listen. Having a therapist who just listens works well when needed, but I was looking for someone who would push me to think differently and challenge me to make changes and grow as a person. Tim gave me homework and always had ideas on how I could try to change or look at things differently. He has great insight, is kind and is organized in terms of scheduling, email responses, sending superbills etc. I recommend Tim to those looking to make a positive change in their lives. Thanks Tim!

Dr. Yen is wise, intuitive, insightful, and compassionate beyond words. As an elementary school teacher, I am faced with a myriad of challenges regarding children behavior and psychology. Dr. Yen has been a "go to" doctor for council and wisdom as I navigated children's behaviors in a classroom setting. When parents ask for references, I refer them to Dr. Yen, because I know they will be well taken care of emotionally, spiritually, mentally, psychologically, and physically.

I had a lifesaving experience with Tim. We met so many different counselors before for my son but none of them was working but he treated my son like a brother with great counselling, and he got so much better within a year. Now he has stopped taking depression medicine and almost back to normal life. If you have a child with depression, I highly recommend him.
